Instructions to fill the Registration Requisition Form : |

- The first column should be kept free. It will be filled by Registration authority in the Office.
- Please fill up your Name and Address in CAPITAL LETTERS carefully with Black ink pen in the second column of this form which will be taken for Registration Process.
- Please sign on the specified place provided to customer (i.e. Signature of the Customer).
- Thumb Impression should be from Left-hand only for Male and Right-hand only in case of Female.
- Thumb Impression should be from Black Ink Pad only.
- Please Paste the Passport size Black & White Photograph Beside's the Thumb Impression.
